The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) GS-7 is to perform more complex nursing assignments within their practice setting. Possesses in-depth LPN/LVN nursing knowledge and skills in specialized procedures and practices. Work is of considerable difficulty and responsibility is performed under general supervision.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d) - and 7407(d)
Licensure: Must possess a full - active - current - and unrestricted licensure as a licensed practical or vocational nurse in a State - Territory or Commonwealth (i.e. - Puerto Rico) of the United States - or District of Columbia
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant

Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment
VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160 - enacted November 21 - 1983 - and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576 - enacted October 28 - 1986
and for their future personnel actions under this authority
The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions
Individuals granted a license by a jurisdiction that does not require graduation from an approved school

GS7-LEVEL Qualifications: Knowledge and skills necessary to care out more specialized patient care duties - including assignments in more complex inpatient and/or outpatient care settings
Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery making astute observations of subtle - less obvious changes (both physical and emotional) in patient's conditions and taking the appropriate action
Ability to proactively assist the RN or MD/OD in addressing patient's needs for medication/treatments - accurately administering prescribed medications/treatments and appropriately recording patient's response to medications/treatments received
Knowledge and skill to promptly recognize potential urgent or emergent patient care situations
Assists individuals or groups of patients and their families to take an active role in promoting health lifestyles for more positive outcomes
Exercises skill in influencing and communicating with unusually difficult to care for or communicate with patients and/or family members who may exhibit problems with lack of self-control - resistant or abusive behaviors - or impediments in their ability to understand or follow instructions
Exhibits skill - creativity - and initiative in contributing to the improvement of overall care delivery and patient and/or family satisfaction levels
Possesses comprehensive practical nursing knowledge and skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care
Supports the RN or MD/DO in monitoring the practice of unlicensed nursing or non-medical staff in patient care delivery
Work of considerable difficulty and responsibility is performed under general supervision
Individuals in these positions are - to a considerable extent - expected to exercise independent technical judgments and to possess and apply a comprehensive working knowledge of the principles - art and science of practical nursing

Preferred Experience: Experience working in a podiatry - wound care - or High-Risk Foot Care (HRFC) clinic setting
Experience performing or assisting with podiatric procedures such as wound management - keratosis reduction - matrixectomies - or orthotic applications
Experience providing nursing care to adult and geriatric patient populations - particularly those with diabetes - peripheral vascular disease - or neuropathy
Experience working independently with minimal supervision - demonstrating clinical judgment and decision-making in a specialty care environment
Experience with electronic health record (EHR) documentation - preferably within the VA/VHA system (CPRS/Vista)
Experience with Reusable Medical Device (RMD) handling - cleaning - and reprocessing per established protocols
Experience working in a multi-site or CBOC environment

Performs podiatric procedures including foot examinations - wound management - keratosis reduction - and suture removal following established clinical protocols
Identifies signs of infection - impaired circulation - or neurologic changes in the feet - reporting findings to the provider for further evaluation and management per established clinical protocols
Assists the Podiatrist with office-based procedures such as matrixectomies - biopsies - verruca removal - casting for offloading - and orthotic applications
Collaborates in the planning - implementation - and evaluation of individualized care plans - coordinating with providers and specialty services to ensure continuity of care
Serves as leader or co-leader in a nursing unit or for a specifically identified group of patients - providing direction - coordination - and clinical oversight in collaboration with the interdisciplinary team
Functions under the general supervision of an RN - MD - or Podiatrist - demonstrating independent technical judgment and broad clinical expertise within the High-Risk Foot Care (HRFC) clinic
Provides patient education based on established clinical protocols related to foot care - limb preservation - and disease management
Ensures accurate - timely documentation in the electronic health record (EHR).Contributes to quality improvement activities by tracking and analyzing infection rates - wound healing outcomes - and amputation trends
Maintains compliance with VA policies for handling - cleaning - and reprocessing Reusable Medical Devices (RMD) - ensuring equipment functionality - readiness - and patient safety
Maintains required certification and specialized training in High-Risk Foot Care (HRFC) per VA/VHA policy to ensure competency and promote optimal limb preservation outcomes
Rotates across Mann-Grandstaff VA locations - including but not limited to the main facility - Spokane Valley CBOC - and Coeur d'Alene - Idaho CBOC - reporting to the assigned site per scheduled rotation
