About the Opportunity
The NP will manage follow-up visits for established patients, allowing cardiologists to focus on new referrals and complex cases. The NP will also support transitional care efforts and provide timely post-discharge visits for patients with heart failure. A key part of this role includes educating patients on cardiovascular conditions, treatment plans, and lifestyle modifications to support long-term health.
Key Responsibilities:
· Conduct follow-up visits for established cardiology patients
· Support transitional care management through post-discharge visits
· Provide patient education and promote medication adherence
· Collaborate with cardiologists and care teams to ensure continuity of care
· Document patient encounters in Epic

Requirements
Education:
· Graduate of an accredited Nurse Practitioner (NP) program
Licensure & Certifications:
· Current California NP license
· DEA Controlled Substance Registration Certificate (CSRC)
· Current BLS and ACLS certification
Experience:
· Experience in cardiology or internal medicine preferred
· New graduates with a strong interest in cardiovascular care are welcome to apply
