Salary: $135,004 – $202,555 (based on experience)
Incentive Bonus: 17.5%

This is a high-impact leadership opportunity for an experienced Case Management leader to oversee operations across a large, high-acuity acute care hospital. You will take ownership of care coordination, discharge planning, and resource utilization across a complex, fast-paced environment, driving outcomes through strong interdisciplinary collaboration and data-driven performance improvement.

Overview

Reporting to the VP of Case Management, this Director will lead all aspects of Case Management, Social Work, and Chaplain services across a 790-bed, Level II Trauma facility. This is a visible leadership role responsible for aligning clinical and operational strategy, optimizing patient flow, and ensuring high-quality, compliant care coordination across the continuum.

What You Will Do

• Provide strategic and operational leadership for Case Management, Social Work, and Chaplaincy services
• Oversee day-to-day operations including staffing, workflows, and performance management across ~110 FTEs
• Lead and develop a leadership team (4 Managers, Social Work Supervisor, Administrative support)
• Drive care coordination and discharge planning processes to ensure timely, compliant, and patient-centered outcomes
• Partner closely with executive leadership, physicians, and post-acute providers to improve throughput and resource utilization
• Analyze operational and clinical data to identify gaps and implement sustainable performance improvement initiatives
• Ensure compliance with regulatory standards, policies, and best practices
• Collaborate with CFO and system leadership on departmental goals, financial performance, and strategic initiatives
• Support a high-performing culture focused on accountability, communication, and continuous improvement

What You Bring

• Bachelor's degree required; Master's preferred (Nursing, Healthcare Administration, or Business)
• RN, LCSW, or LMSW required
• 3+ years of acute care hospital Case Management experience
• 2+ years of leadership experience in Case Management
• Proven ability to lead large, complex teams and drive measurable outcomes
• Strong analytical skills with the ability to translate data into actionable plans
• Executive presence with excellent communication and relationship-building skills
• Experience collaborating with physicians and post-acute networks
