Under the direction from the supervising social worker, the incumbent will work with individuals living with HIV (or affected by HIV) and families to provide low threshold/low barrier social work services within an ambulatory care setting. These services include, but are not limited to: psychosocial assessments, assisting patients in referrals to emergency and long-term housing stabilization, substance use counseling, mental health assessments, and appropriate referrals to community agencies for financial benefits, legal, and immigration resources.
