The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking a highly skilled & experienced Nurse Practitioner (NP) to serve in our Home Base Primary Care (HBPC) Service at the Ventura CBOC. The NP will assess - coordinate - facilitate & deliver nursing care for patients utilizing the nursing process. Provides direct & indirect nursing care to patients & families. Accountable for adhering to established polices for standards of care & standards of practice.
Duties
The Home Based Primary Care (HBPC) program provides interdisciplinary - comprehensive health care services to Veterans in their home
HBPC patients are often older adults with multiple chronic medical conditions for whom travel outside the home to access clinic-based care may be difficult
The Nurse Practitioner (NP) is responsible for assessing - diagnosing - planning - implementing - and evaluating direct and indirect health and quality of care for patients under their care
The NP will drive a government vehicle from the VA campus to the homes of patients to provide them with primary care in the home setting
Some patient care may be provided virtually (e.g
via telephone or video visits)
Position is located at the Captain Rosemary Bryant Mariner Outpatient Clinic in Ventura - CA
Although the position is based out of the Ventura Outpatient Clinic - the NP will also be required to see patients occasionally outside of their assigned HBPC catchment area and be required to cross-cover for other GLA HBPC providers
Required training may occur at any of the other HBPC locations (Sepulveda - LAACC - West Los Angeles)
Duties include but are not limited to: Provides direct and indirect patient care - and works as a member of a healthcare team to oversee and coordinate medical care for a population of Veterans in the given practice setting
Takes comprehensive histories - provide physical examinations and other health assessment and screening activities - diagnose - treat - and manage patients with acute and chronic illnesses and diseases
Orders laboratory and imaging studies and integrate the results into clinical decision making
Provides care management and clinical procedures and tasks consistent with licenses
Prescribes medication and durable medical equipment
Makes appropriate referrals for patients and families - and request consultations
Aids in health promotion - disease prevention - health education - and counseling as well as the diagnosis and management of acute and chronic diseases
Attends staff meetings - team meetings - committee meetings - and completes peer review evaluations
Provides leadership in identifying and addressing ethical issues that impact patients and staff
Manages program resources (financial - human - material - or informational) to facilitate safe - effective - and efficient care
Implements educational plan to meet changing program or service needs for self and others
Uses professional standards of care and practice to evaluate programs and/or service activities
Consistently demonstrates positive effective customer service - communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers
Provides active leadership in program-based performance improvement activities
Participates in support of educational training of graduate students and other Allied trainees in related-fields
Collaborates with others in research activities to improve care
