The COMPACT Act Social Worker Program Coordinator functions as a coordinator for the care of Veterans at risk who have eligibility through the COMPACT Act. They serve as an advisor to facility leadership - program managers - and staff concerning the COMPACT Act and strategies for individual Veterans - the facility - and the Veteran community.
Qualifications
To qualify for this position - applicants must meet all requirements within 30 days of the closing date of this announcement.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited
A doctoral degree in social work may not be substituted for the master's degree in social work
Licensure
Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level
English Language Proficiency
Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f)
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria)
Grade Determinations: Social Worker (Program Coordinator) - GS-12 Experience and Education
One year of experience equivalent to the GS-11 grade level
Experience must demonstrate possession of advanced practice skills and judgment - demonstrating progressively more professional competency
Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship - or equivalent supervised professional experience
Licensure/Certification
Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level - and must be able to provide supervision for licensure
Demonstrated Knowledge - Skills - and Abilities
In addition to the experience above - the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation - negotiation - and monitoring
Knowledge and ability to write policies - procedures - and/or practice guidelines for the program
Ability to supervise multidisciplinary staff assigned to the program
Skill in organizing work - setting priorities - meeting multiple deadlines - and evaluating assigned program area(s)
Ability to provide training - orientation - and guidance within clinical practice

Duties

Responsible for coordination between health care providers and care managers in direct care and community care
Responsible for comprehensive transition planning
Responsible for outreach to community providers and healthcare systems providing COMPACT care
Advocates for Veterans working closely with fiscal and business units to assure they do not receive bills for COMPACT-related care
Serves as a singular point of contact for Veterans - families - and caregivers during COMPACT episodes of care
Serves as the facility subject matter expert for the COMPACT Act
Coordinates with local Patient Advocate Tracking System (PATS-R) COMPACT clinical review teams
Collaborates with local facility revenue and finance departments
Coordinates with their local facility Office of Integrated Veteran Care (IVC) Coordinators
Works with Veterans at high risk for suicide
Screening/Assessment - Develops an assessment of the Veteran independently and/or in collaboration with the interdisciplinary treatment team - the Veteran - family members - and significant others
Makes a diagnosis using the Diagnostic and Statistical Manual of Mental Disorders
Provides goal planning - evaluation - and intervention with serious and complicated - highly diverse cases involving substance abuse and psychiatric illness
Treatment Planning/Goal Setting - Develops or contributes to the development of the mental health treatment plan and setting achievable treatment goals with the Veteran and/or family and with the interdisciplinary treatment team
Provides consultation and makes recommendations to the interdisciplinary team on the course of treatment
Referral to Service Providers - Collaborates with other service providers in reassessing the Veteran's needs
Responsible for educating the Veteran and/or caregiver of the available services and assisting them with appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker related to mental health issues
Crisis Intervention - Assesses for suicidality and self-harm and provide appropriate intervention including but not limited to safety planning
Coordination of Care - Reviews the progress notes from other providers to accurately determine the strengths - limitations - and needs of each Veteran being referred for mental health services
Psychotherapy - Provides Veterans with psychotherapy to treat psychiatric diagnoses and mental health symptoms
Provides psychotherapy to individuals and families as well as group therapy
Performs as a highly developed professional - with the knowledge and experience that is required in working with the full range of mental health symptoms and diagnoses - including chronic and severe mental illness - mood disorders - anxiety disorders - personality disorders - substance use disorders - and dual diagnosis
Case Management - Provides comprehensive case management services
These services include but are not limited to - assistance with housing - income and benefits - community programs and services - and VA programs (Vocational Rehabilitation - Mental Health Day treatment - substance abuse treatment - transitional lodging - travel - primary care - etc.)
