This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited
A doctoral degree in social work may not be substituted for the master's degree in social work
Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level

English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f)
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria)
Grandfathering Provision: The following is the standard grandfathering policy for all title 38 hybrid qualification standards
Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation
All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title - series and grade held - including positive education and licensure or certification that are part of the basic requirements of the occupation
For employees who do not meet all of the basic requirements in this standard - but who met the qualifications applicable to the position at the time they were appointed to it - the following provisions apply: Such employees in an occupation that requires a licensure or certification - may be reassigned - promoted up to and including the full performance (journey) level - or changed to lower grade within the occupation - but may not be promoted beyond the journey level or placed in supervisory or managerial positions
Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed - on a temporary or permanent basis - until they fully meet the basic requirements of the standard
Employees initially grandfathered into this occupation - who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation
If an employee who was retained in an occupation listed in 38 U.S.C. 7401(3) under this provision leaves that occupation - the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation
NOTE: If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS-0185 social work series - they lose the grandfathering protection
If they choose to return at a later date to the GS-0185 series - they must be licensed or certified to qualify for employment as a social worker
Grade Determinations: Social Worker (Program Coordinator) - GS-12: Experience and Education: One year of experience equivalent to the GS-11 grade level
Experience must demonstrate possession of advanced practice skills and judgment - demonstrating progressively more professional competency
Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship - or equivalent supervised professional experience
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level - and must be able to provide supervision for licensure
Demonstrated Knowledge - Skills - and Abilities: In addition to the experience above - the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation - negotiation - and monitoring
(b) Knowledge and ability to write policies - procedures - and/or practice guidelines for the program
(c) Ability to supervise multidisciplinary staff assigned to the program
(d) Skill in organizing work - setting priorities - meeting multiple deadlines - and evaluating assigned program area(s)
(e) Ability to provide training - orientation - and guidance within clinical practice
Assignments: For all assignments above the full performance level - the higher-level duties must consist of significant scope - complexity (difficulty) - and variety and be performed by the incumbent at least 25% of the time
Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area - such as but not limited to: Spinal Cord Injury - Homeless Continuum Veteran Program - Hospice and Palliative Care Program - Suicide Prevention Program - Veterans Justice Outreach - Caregiver Support Program and Community Nursing Home Program
The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area
The program coordinator oversees the daily operation of the program - develop policies and procedures for program operation - and prepare reports and statistics for facility - VISN - and national use
They may be responsible for the program's budget - developing and monitoring staff compliance with practice - standards and guidelines on documentation - workload - data entry - ethical practice and service delivery
The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service
They may be responsible for - or contribute to - the program's resource and fiscal management - monitoring control points developing the annual budget - operating within that budget - and accounting for appropriated funds
The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility - VISN - and national use
They provide leadership - direction - orientation - coaching - in-service training - staff development - and continuing education programs for assigned staff
They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports - including productivity assessments
They oversee program operations and evaluations - identifying areas for improvement - gathering relevant data - assessing the data - developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.]
Duties
Major duties include but not limited to: The social worker independently performs outreach and facilitates eligibility determination by securing military discharge papers when necessary and facilitating enrollment for care with VA eligibility staff and determining HCHV eligibility in accordance with the HUD Mc Kinney Act definition of homelessness
The social worker independently conducts psychosocial assessments
develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team
The social worker possesses the knowledge and skills to independently implement treatment modalities and provide counseling or psychotherapy for individuals - families and groups
The social worker coordinates care with multiple providers - including mental health - medical - and other providers
The social worker possesses expert level skills to perform psychosocial interventions and measure outcome results
The social worker coordinates the clinical care for a limited number of contracted transitional housing programs
The social worker coordinates contract transitional housing admissions and discharges for a limited number of programs
The social worker is proficient in coordinating community based services - including information and referral for additional services from other VA programs - other government programs and community agency programs
The social worker independently identifies high risk patients with complex or serious health and/or mental health problems and provides case management services
The social worker provides crisis intervention services - seeking to address the cause as well as presenting complaint - engaging family and other support systems as appropriate
The social worker has functional knowledge and experience in the use of medical and mental health diagnoses - disabilities and treatment procedures - including acute - chronic and traumatic illnesses and injuries - common medications and their effects - side effects - and medical terminology
Has the ability to identify multiple causes of physical and/or mental illnesses when developing treatment plans
The social worker establishes and maintains effective therapeutic relationships with Veterans and their families when appropriate
The social worker works independently with patients and families who are experiencing a variety of psychiatric - medical and social problems - utilizing individual - group and family counseling and therapy skills
The social worker independently coordinates consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans
The social worker is able to communicate effectively - both orally and in writing - with people from varied backgrounds
The social worker has general knowledge of Veterans' benefits and services relating to special programs - service connected compensation and non-service connected pension
The social worker coordinates referrals to these services based upon Veterans' needs and eligibility
The social worker has working knowledge and skill in the use of word processing - data management and other computer systems - especially those programs in use by the VA
The Social Work Case Manager will be able to meet the needs of customers while supporting VA missions
Consistently communicates and treats customers (Veterans - their representatives - visitors - and all VA staff) in a courteous - tactful - and respectful manner
The social worker is able to provide the customer with consistent information according to established policies and procedures
Handles conflict and problems in dealing with the consumer constructively and appropriately
Has working knowledge and skill in the use of word-processing - data management and other computer systems - especially those programs in use by VHA
The Social Work Case Manager will fulfill ethical responsibilities to Veterans - colleagues and VA in accordance with relevant discipline Code of Ethics - VHA Code of Ethics - and local Ethics policies - which includes but is not limited to compliance with all laws and regulations related to confidentiality and release of information - and protecting Veterans' personal identifying information
Documents in the patient's medical record in accordance with medical center policies - and the requirements of JCAHO and CARF
This may include a discipline-specific screening and/or development of a biopsychosocial assessment and treatment plan within the time frame required by professional service standards
