The LVN utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio-economic background and response to medical regimen and treatment. Under the direction of the RN Navigator, s/he will assist with review of new patient referrals, triage provider in basket messages, and assist with rooming patients as needed. This individual will utilize his/her good customer service and communication skills on a daily basis via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers.
