Primary Purpose
Provides strategic leadership for all components of the system-wide Patient Safety program incorporating High Reliability principles and day-to-day leadership / oversight of the Patient Safety department. Actively participates in the development, implementation and monitoring of Clinical Risk Management/Patient Safety policies, procedures, and initiatives to continually improve the system Clinical Risk Management/Patient Safety program. Advocates for and nurtures a system – wide culture of safety for patients, workforce members, providers, and visitors through direct interaction.
Minimum Specifications
Education
Requires a master’s degree in healthcare or healthcare related field.
Experience
Must have 8 years of professional experience in the area of Patient Safety and/or Clinical Risk Management or a combination among the two specialties equal to 8 years; two (2) of those years would require management experience.
Equivalent Education and/or Experience
Experience in a highly complex system may substitute for the years of experience listed above.
Certification/Registration/Licensure
Must have one of the following certifications upon hire: Certified Professional in Patient Safety (CPPS) from the Institute for Healthcare Improvement (IHI) or the Certified Professional in Healthcare Risk Management from the American Society for Healthcare Risk Management (ASHRM).
For RNs: Must have current unrestricted RN license or temporary license with the Texas Board of Nursing or valid compact RN license, or ability to obtain within 90-days of hire. Must have current CPR course (Healthcare Provider course for RNs; Heartsaver Course for non-RNs) completion card from one of the following:

Must have documented experience of professional leadership of Patient Safety Clinical Risk Management in a large hospital system.
Must demonstrate proven performance and expertise in leading Root Cause Analysis (RCAs) and Failure Mode Effects Analysis (FMEAs), using a variety of tools, including but not limited to Cause Mapping, Process Flow Mapping.
Maintains nationally recognized certification in Patient Safety or Patient Safety Clinical Risk Management.
Maintains standards of professional conduct and performs all assigned and unassigned duties in accordance with the current State of Texas Nursing Practice Act.
Must demonstrate the ability to communicate effectively both oral and written.
Must demonstrate effective facilitation skills for small, medium, and large projects.
Must demonstrate organizational, time-management, analytical and problem-solving skills.
Must demonstrate working knowledge of MS Office software to include Word, Excel, PowerPoint, and Outlook.

Responsibilities
Oversees the operation of the Patient Safety & Clinical Risk Management department using High Reliability and ICARE principles to foster and establish a highly reliable Patient Safety culture. Leads development and implementation of standardized policies, procedures, practices, and standards system-wide.
Actively participates in the development, implementation and monitoring of Clinical Risk Management/Patient Safety policies, procedures, and initiatives to continually improve the system Clinical Risk Management/Patient Safety program.
Advocates for and nurtures a system – wide culture of safety for patients, workforce members, providers, and visitors through direct interaction.
Demonstrates leadership in Clinical Risk Management Patient Safety through full integration of High Reliability and ICARE principles incorporating the Parkland mission and values across the system
Maintains oversight for the system-wide event reporting system including the implementation and ongoing maintenance / operation of the system.
Leads and actively participates in the facilitation of Failure Modes and Effects Analysis process for the system to identify failure modes and create mitigation strategies.
Directs the system-wide investigation and response to patient Quality of Care grievances for all services within the system.
Maintains oversight of the investigation of actual or potential serious reportable events including conducting Root Cause Analysis across the system. Facilitates disclosure of facts after an actual or potential adverse event to patients and families as needed. Facilitates bill adjustments and accurate coding of significant adverse events according to policies.
Coordinates with the system Legal department to ensure efficient processes exist for responding to external information requests relating to potential or actual lawsuits, Public Information Act (PIA) requests and other issues that arise.
Develops and maintains positive working relationships with providers, management, and staff to promote open communication and accurate flow of information. Maintains communication related to significant hospital events and informs senior leadership, corporate communications, and Legal in conjunction with the Vice President for Quality and Safety and the Sr. Vice President of Quality, Safety & Performance Improvement as deemed necessary.
Assesses and recommends data collection of patient safety and clinical risk data for the health system in order to identify trends and opportunities for improvement.
Participates as an active member on safety and clinical risk related medical staff and hospital committees.
While respecting and supporting the system financial goals, selects and nurtures appropriate staffing levels, including areas of expertise to enhance the departments’ ability to mitigate clinical risk and promote patient safety.
Provides oversight of patient safety adverse events within the system, ensuring transparency in investigations and thoroughness for maximum effectiveness.
Stays abreast of the latest developments, advancements, and trends in the field of Patient Safety Clinical Risk Management by attending seminars / workshops, studying professional journals or literature, actively participating in professional organizations, and maintaining certification / licensure. Translates the knowledge into continual improvement of the departments’ work processes and patient safety activities across the health system.
Promotes High Reliability both within the department, division, and across the system by actively participating in system initiatives driving high reliability. Serves as High Reliability Champion for the system.
Develop and monitor the annual budget for the department ensuring the department has sufficient funding to carry out the goals and objectives set for the department in support of the system goals.

Makes recommendations for disciplinary action as necessary, up to and including termination, to ensure maximum utilization of individual and group capabilities. Ensures that assigned employees receive opportunities to further their knowledge and professional growth.
