The Care Manager provides guidance and facilitation to assist students in optimizing their educational experience while minimizing the impact of illness, injury, and personal distress on students' academic careers through the coordination of care, interventions, and social support with appropriate referrals, and follow up services within a broad network of campus and community resources. The scope of this position is to collaborate in providing support to students in distress by triaging concerns and coordinating a response from the college community. Care Manager may oversee professional referrals for both on and off-campus providers, on-going support, outreach, and follow up to undergraduate college community. The position works closely with members of the Care team, to assist with various aspects of case management administration regarding triage/intake/crisis systems and quality assurance.
In collaboration with faculty, administrative departments, and families where appropriate, the Care Manager seeks to provide a safe, welcoming and affirming environment for all students. Under the direction of the Lead Care Manager, provides case management and support in collaboration with the Division for Student Life team for students in complex needs.
The expected hiring range for this position is USD $27.93 to USD $35.61 , based upon the position's scope and responsibilities.
