If hired for this position your role would include developing and carrying out patient management plans within the context of the interdisciplinary team, providing effective counseling and education to patients and their families to facilitate patient-focused and value driven healthcare and promoting informed decisions about diagnostic and therapeutic interventions based on patient information and preferences.
