Your day.

As a Home Health Patient Care Tech / CNA, you need to know how to:

• Work effectively using keen observation and communication skills in an independent environment utilizing established reporting structure to interdisciplinary healthcare team. Recognize signs and appropriately address within scope of practice areas of physical pain, spiritual or emotional distress and report signs to the nurse. Recognize emergencies and respond swiftly and safely.


• Manage patient assignment and schedule effectively and efficiently by organizing the schedule for visits and notifying patients and families of planned schedule and any changes as necessary. Notify supervisor of delays or barriers as appropriate.


• Maintain flexibility in assignment and geographic locations: be willing and able to drive distances within service area in variable conditions, be willing and able to remain in location to provide short term respite or hospice continuous care (when floating to hospice division), participate in orientation of other CNAs, and participate in joint team visit as required.


• Demonstrate enhanced communication skills including empathy, calmness, active listening and emotional support for patients and family members particularly during high stress situations (complex medical care, functional decline with disease progression, pain crisis, seizure activity, dying process, etc.).


• Assist the patient/client with the tasks for activities of daily living as outlined in a written care plan (ambulation, personal care, transfers and positioning, meals, toileting, self-administered medication assistance, simple dressing changes, non-complicated ostomy care, light housekeeping, vital signs, light housekeeping and occasional light meal preparation).


• Demonstrate ability to reinforce and explain basic personal care tasks as performed and to assist in-home caregivers i.e.: basic assisted hygiene, safe transfers, ambulation, repositioning and turning patients.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

• Successful completion of an accredited nursing assistant training program


• Certification as a Nursing Assistant (CNA) in state of practice


• Current BLS certification endorsed by the American Heart Association or the ability to obtain within 60 days of hire (with prior approval)


• Minimum of one (1) year of experience in a similar setting


• Reliable automobile, current unrestricted driverâ™s license, and automobile insurance

Preferred:

• High School Diploma or equivalent
