The Clinical Documentation Specialist (CDS) is responsible for performing concurrent review and analysis of the clinical facts documented by the physician in the medical record. The CDS facilitates and obtains appropriate physician documentation for any clinical conditions or procedures to support the appropriate severity of illness, expected risk of mortality, and complexity of care of the patient. The CDS exhibits appropriate knowledge of clinical documentation requirements, DRG assignment, and clinical conditions / procedures. The CDS coordinates with the physician advisors, Medical Director, and Director of CDI in assisting with the education of the medical staff regarding accurate and complete physician documentation
