The OHSU Family Medicine at Richmond (FMR) clinic is a Federally Qualified Health Center (FQHC) with the following mission statement :To provide high-quality, comprehensive healthcare services for the community of South East Portland, especially its vulnerable residents, while teaching healthcare professionals. OHSU Family Medicine at Richmond Clinic patients are generally lower socioeconomic, inner city residents. Many are immigrants. This patient population includes high levels of patients with mental health, substance use, cardiovascular illness, diabetes, and depression needs. 15% of patients speak languages other than English includingVietnamese, Chinese, Russian, and Spanish. In alignment with OHSUs commitment to address institutional racism all OHSU Family Medicine at Richmond (FMR) employees will actively and enthusiastically The OHSU Registered Nurse (RN) provides compassionate, evidence-based, and efficient care to individuals, families, communities, and patient populations. This care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics and meets the standards/expectations of a Professional Practice Model. In that model, the nurse demonstrates the professional role obligations of scientist, leader, practitioner, and knowledge transferor. Professional accountability enriches the nurses engagement as a leader in promoting an interprofessional environment, collaborative decision making, innovation, life-long learning, and teamwork. The Nurse Care Manager at OHSU Family Medicine at Richmond (FMR) will provide quality nursing care to patients consistent with the FMR care philosophy:patient-centeredness, advanced access, non-judgmental, harm reduction, providing a robust medical home for vulnerable patients, behavioral health integration, team-based care, and empanelment. The nurse serves in a clinical leadership role to the care team with primary responsibilities for disease management, higher-level care coordination, and advice/triage needs for the care teams panel of patients.The nurse ensures that the team functions smoothly and efficiently while keeping the patients well-being paramount. The Nurse Case Manager will set a positive example of nursing for the Family Medicine residents, staff, and visiting students; and conduct their self in a friendly, courteous and professional manner when involved with any activity associated with OHSU. Working conditions: Constant exposure to usual hospital environmental conditions including possible exposure to blood and body fluids for which staff is provided education and protective equipment to minimize risk.
