You’re comfortable to independently manage, coordinate care, and develop education for specific patient populations being evaluated for advanced cardiovascular disease, procedures and treatments.

Your day.

As a RN CV Program Coordinator, you need to know how to:

• Exercise clinical competence in the hospital and continuum setting including but not limited to a comprehensive evaluation, interpretation of diagnostic data, assessment, coordination of patient care and interpretation of outcomes data.
• Actively participate in patient care rounds and respond to any urgent patient care activations to provide support and identify any education and training needs for the care team.
• Communicate effectively providing instruction and explanation to the patient and/or significant others, explain nursing intervention, communicate with all members of the healthcare team.
• Collaborate with the director to oversee and ensure that the program meets the regulatory requirements. Perform chart reviews and collect, analyze and report data for performance improvement activities, planning and evaluation of the program.
• Work collaboratively with the providers, director and care team to develop and implement evidence-based policies and standards for the program.
• Assist patients and families with navigating through the healthcare system
• Assess plans, develop, implement and evaluate activities that promote professional development and continuing competence of care site providers and associates.
• Consult regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.
• Provide regular educational offering to patients and members of the healthcare team.
• Maintain and/or analyze data collected on assigned program. Serve as the leader and subject matter expert for the program.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

• Current license to practice nursing in the state of Montana, required.
• Current BLS certification endorsed by the American Heart Association, required.
• Current ACLS certification, required.
• Minimum of three (3) years of relevant RN clinical experience  in a Cardiovascular service, required.
• Demonstrated positive working relationships with Cardiologists, Electrophysiologists, Cardiothoracic and Vascular Surgeons and Internal Medicine physicians, required.
• Bachelor’s Degree in Nursing (BSN), preferred.
