At Denver OB-GYN, we provide obstetrical and gynecological care to adolescents, women, pregnant women and the elderly, and because we are located on the campus of Saint Joseph Hospital, we deliver babies at their Center for Women and Infants. As a Care Manager Social Worker you need to know how to:
Provide triage, assessment, and counseling for Seton Women’s Center patients as identified as needing behavioral health intervention
Conduct psychosocial assessment interviews with patients and their families and collateral caregivers
Develop and implement treatment plans for stabilization and behavioral modification, mobilization and disposition appropriate for patient care
Function as part of a multidisciplinary team to develop patient care plans
Document care management activities and clinical assessments in patient’s medical records as required
Provide services using accepted psychotherapeutic models and interventions
Provide consultation to primary care providers as needed and collaborate with staff and providers on treatment plans
Ensure quality of care for patient by maintaining clinical focus on the restoration of the patient to physical and psychological safety
Provide patients with information regarding community resources and facilitate connections with these resources
Participate in patient-centered departmental and inter-disciplinary meetings as well as non-departmental meetings and ongoing hospital committees as requested
Represent adherence to mission philosophy, vision and objectives of the clinic and the department of Behavioral Health

Master’s Degree in Social Work
Licensed Clinical Social Worker in the State of Colorado
Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
Knowledge and experience with managed care
Previous experience in integrated healthcare or primary care
Experience with PC software applications in word processing, Epic, Google applications
