Responsibilities The Clinical Case Manager will support the interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. This position helps drive change by identifying areas where performance improvement is needed.
Current RN license.
Graduation from an Accredited School of Nursing, BSN preferred.
Minimum of 3 years recent experience in an acute care setting.
Experience with discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
Experience performing in-depth chart analysis.
Extensive experience working in healthcare environment.
Computer skills performing data collection and data entry functions.
Skills in reading and comprehending English text and medical terminology.
Excellent communication (writing and verbal) skills.
Knowledge of medical and nursing treatment plans, levels of care, etc.
Knowledge of Medicare, MediCal, and HMOs, MIA, County, and third party payor contracts.
Knowledge of Title XXII, The Joint Commission, and Lumetra criteria.
Special Requirements This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment. Verification of licensure will be required at time of the appointment.
