The Heart & Vascular Center Nurse Coordinator provides care coordination, education, and resources for the CT Surgery patient population across the continuum of care in collaboration with patient care services, cardiac catheterization lab, interventional radiology, CT Surgery, and the CT Surgery Clinic.
The Cardiothoracic Surgery Nurse Coordinator develops relationships with CT Surgery patients prior to discharge, oversees the discharge process, and is a resource for discharged patients. The Nurse Coordinator participates in quality improvement and service improvement. Applies experience and judgement to determine the importance of a situation, set priorities, and uses abstract thought to evaluate clinical situations. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act. This position reports directly to the Heart & Vascular Center.
Minimum 4 years CT surgery and or/cardiovascular nursing experience in a hospital setting.
Graduation from an accredited School of Nursing.
Possession of current California RN License.
Ability to read, write, speak, and understand the English language.
Knowledge of current trends in nursing.
