Asa Social Worker II Discharge Planner at the Behavioral Health and WellnessCenter, you will optimize the quality of life of all those served by expandingbeyond the traditional focus on medical needs to the needs of the whole personfor the patient with a psychiatric illness.
Whatyou will do in this role:
Youwill be responsible for assessing the psychological, social, cultural and/orfinancial needs that impact the high-risk patient’s health and recovery
Youwill utilize social services, counseling and clinical knowledge in theassessment of these needs
Youwill be responsible and accountable for coordination of services through aninterdisciplinary process which provides a clinical and psychosocial approachthrough the continuum of care and balances individually identified patients andfamily needs with the efficacious and cost effective utilization ofresources
Throughconcurrent case management, you will assess patients to determineappropriateness of admission, continued hospitalization, as well as appropriatelevel of care
Youwill conduct discharge planning that will begin at the time of (or prior to)admission, and reassessed ongoing throughout the course ofhospitalization
Youwill use your knowledge of payer source implications on discharge plan,resources and placement options to maximize patient’s successfuldisposition
Whatqualifications you will need:
Licensed Social Worker (LSW)preferred
Master of Social Work (MSW) fromaccredited school of social work preferred. Master of Arts (MA) and / or (MS) in related mental field or Marriageand Family Therapy (MFT) from appropriate accredited school acceptable
Must be in process of obtainingClinical licensure in specific discipline or open to supervision
Three (3) years minimum of recentCase Management or Social Worker experience in an acute care setting preferred.
Case Management experience in MentalHealth setting preferred
Must have excellent written, verbaland interpersonal communication skills and able to work with highly diversepopulation
Must be knowledgeable of State andJoint Commission standards
Must be able to apply criticalthinking and formulate diagnostic impressions based upon review of systems,history, symptoms and current presentation
Must be able to provide crisisintervention and utilize system resources/referrals
