We are seeking a compassionate, dedicated Nurse Practitioner to execute the following job duties with our Outpatient Advanced Heart population.
As the Nurse Practitioner, you will be responsible for the planning and coordination of all aspects of the work up and care for the patients in the Advanced Heart Failure and Pre Heart Transplant, throughout the continuum of services - from initial referral to follow up care.
You will be responsible for evaluating the patients (includes: chart review, performing history and physical examinations, ordering diagnostic tests, reviewing results and reporting to the Physician as needed.
You will provide Inpatient services to Advanced Heart Failure and Pre Heart Transplant patients, coordinate daily activities of inpatient service, including care coordination/patient education, timely documentation of patient progress notes, discharge summaries, and CMS required care coordination notes.
You will coordinate communication of patient ADT and surgeries with referring and collaborating MDs.
You will develop and implement patient management plans and assist in provision of continuity of care.
You will provide education, leadership and patient care management across the health care continuum from preventative and acute care through discharge.
The Nurse Practitioner will serve as a patient advocate throughout the care process and represent the medical center at regional and national meetings/conferences.
