The Social Work Specialist assists in the development of plans to provide continuity of care and enable patients and families to achieve maximum physical, social and emotional health. This position will serve several neurologic sub-specialties which all use the same clinic space on different days. Typical patient services include the following: screen patient/family psycho-social needs and determine current and anticipated patient care concerns; advocate and support patients and families, provide family with information regarding patient care issues, coordinate medical care, coordinate referral to outside of OHSU providers, help patients and families connect to community services, provide limited case management to Neurology\u2019s vulnerable patient population, and facilitate in the coordination of follow up care in OHSU clinics or in the community.
This position will provide general support to additional clinics as appropriate.
