As an Integrated Behavioral Health Therapist, you will be a key part of our patient centered care, to ensure that behavioral health needs of our client population are recognized, assessed, and treated to improve the overall health and well-being of the patient.

We are currently looking for a full-time therapist to work within our Integrated Behavioral Health Program with both children and adults, which is located within our Medical Clinics. This is a program where the Therapist and Primary Care Physician, along with our consulting Psychiatrist, work together to facilitate positive physical and emotional changes in client’s who are seeking help. We are looking for a flexible candidate, who has good communication skills and the ability to excel in a fast-paced environment.
Maintain an ongoing patient load and conduct short-term therapy sessions meeting productivity standards.
Conduct intake assessments and participate in warm hand offs.
Conduct individual therapy services for patients over the age of 5 and up. Possibly conduct couples, family or group therapy as needed.
Complete documentation using the electronic health record in a timely fashion while adhering to related laws and regulations for documentation.
Track patient progress, utilize screening measures as required.
Monitor and coordinate the delivery of health services for clients as related to integrated behavioral health care, including linking with other treatment providers not only within the primary care setting but, with the clients’ permission, outside it as well.
Assist the primary care provider in recognizing, treating and managing mental Health and psychosocial issues and acts as a contributing member to the primary care team.
Attend weekly IBH staff meeting.
Attend off-site trainings or webinars as directed in order to remain up to date on evidence based techniques and approaches.
Provide all services in accordance with accepted standards of care and Axis protocols within the parameters of agency contractual requirements.
Conduct psychological testing, or assessments as requested. Assist providers in diagnostic clarification where needed.
Wide range of therapeutic techniques, to include: some education/experience with short-term, solution focused therapy approach, CBT and DBT techniques. Flexibility in treatment approach is preferred (Ability to utilize a 30-minute session model and conduct brief treatment where necessary).
Demonstrated ability to maintain a case load of patients and communicate with larger treatment team. Ability to write and maintain treatment plans, develop treatment goals and work as a team to assist patients in meeting goals.
Strong written & verbal communication skills.
Proven ability to build relationships with patients and staff; must be service oriented.
Computer knowledge and ability to type is required. Previous experience using electronic health records is a plus.
Excellent time management skills to meet goals and objectives.
Ability to multi-task effectively; organized and dependable.
Ability to interact effectively and in a professional, supportive manner with persons of all within a fast-paced medical clinic environment.
Experience providing trainings, supervision, or leadership role preferred.
Bilingual capacity (Spanish, Hindi, Farsi) is highly desirable.

Axis Community Health began in 1972 as a health center for low-income children in Eastern Alameda County who had no other access to health care. Over the past forty-five years, Axis has grown to provide medical care for all ages. We have also added mental health counseling services, addiction recovery programs, court-ordered programs, a WIC nutrition program and school and community-based health education services. Until February 2004, we were known as Valley Community Health Center. We changed our name to Axis Community Health in an effort to increase our community visibility and eliminate the confusion that existed in the community between our organization and ValleyCare Medical Center, the local hospital. Axis now offers services from five sites in the Tri-Valley area and serves more than 14,000 members of the community.
