The Coordinator Patient Safetywill assist the Patient Safety Manager (PSM) in providing leadership andexpertise in the delivery of safe patient care. In doing so, theCoordinator Patient Safety will coordinate information obtained from thereporting of adverse events, near misses and concerns related to patientsafety. The Coordinator Patient Safety will coordinate multiple projectswithin a multidisciplinary team and gather necessary data. Essential functions of the role includeassisting the PSM with the following:
Â§Reviewingdata from Adverse Events (Midas SSOS) to support and monitor the effectivenessof fall reduction programs specifically targeted to patient populations at highrisk.
Â§Designof policy and practice changes that will improve the medication safety programand patient safety goals
Â§Coordinatespatient safety metrics and relevant educational materials / training.
Â§Creatinga safe culture by sharing accountability for patient safety throughout thefacility.
Â§Reviewinglegislative and regulatory activities related to patient safety.
Â§Servesas backup for the following committees: Patient Safety Committee, OccurrenceReporting, Environment of Care Safety, HW Best Care Communications PracticeCouncil, Infection Control Committee, Medication Management Committee, NursingQuality and Patient Safety Committee, Nursing/Lab Collaboration Committee, TubeSystem Users Group, Safety First Advocates Committee, Joint CommissionAmbulatory Subcommittee, and Joint Commission Readiness Committee.
Â§Coordinationand reporting of National Patient Safety Goals (NPSG), Global Trigger Tool(GTT), The Leapfrog Group, Patient Safety Database, PS Survey, PS Site Visit,and other PS initiatives via surveys and audits.
Â§Coordinationof process improvement projects and task forces.
Â§Servesas backup for PS TalkRounds, The Joint Commission Mock Surveys, and RCAs.
Â§Recognizinghospital employees for reporting near misses and Stop the Line events.
The ideal candidate for theCoordinator, Patient Safety role is a Registered Nurse with a Bachelorâs degreeand 3+ years of experience.
