Description The Pharmacy Department provides comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, pharmacy technology, and teaching) throughout the Health System 24 hours per day. The Department is comprised of three inpatient, six outpatient pharmacies, a sterile production lab, and the full complement of pharmacy purchasing, stores and distribution.

Qualifications REQUIRED -California Board of Pharmacy License
PREFERRED -3 years or more experience working in ambulatory care clinic setting -Completion of Pharmacy Practice PGY1 or PGY2 residency -BPS or CSP certification -Experience developing ambulatory care clinical services -Experience working with Neurology clinic patients and specialty medications -Experience precepting and evaluating residents and students

Responsibilities Pharmacists are integral members of the health care team at UC Davis Health. They share responsibility for comprehensive medication management, distribution of medications to inpatients and ambulatory patients, oversight and development of treatment guidelines and formulary management, stewardship of high-cost drug therapy, teaching for pharmacy students, residents and staff, clinical research, participation in Department and Hospital Committees, task forces, strategic planning and key initiatives related to pharmaceutical care..
This position will work in the Specialty Clinics and Specialty Hub/Pharmacist-based Prescription Renewal Clinic to provide services to patients and assist patients in getting their medications. The pharmacist's responsibility is to monitor drug therapy to achieve desired outcomes and to avoid drug toxicity and the dosing of medication based on patient demographics, available treatment guidelines and response. The pharmacist works as an integral part of the patient care team and assists physicians and nurses in providing drug therapy.
