Job Summary Coord, Patient Education will instruct patients and families on relevant topics according to assessed learning needs in a clear and professional manner. Position will assist medical staff with the selection and/or production of patient education programs that achieve desired results. Responsible for seeing all inpatients on hospital day 2 to assess patient needs and satisfaction. Essential Functions * Instructs patients and families on relevant topics according to assessed learning needs in a clear and professional manner. * Assists medical staff with the selection and/or production of patient education programs that achieve desired results. * Sees all inpatients on hospital day 2 to assess patient needs and satisfaction. * Conducts administrative duties for the hospitalist groups as instructed by hospitalist medical director. * Performs discharge phone calls for selected patients. * Performs daily census update. * Maintains encounter data for hospitalist group. * Checks and sorts abnormal results and notifies patient as needed. * Faxes discharge summaries to non-Scott & White physicians. * Receives and addresses patient phone calls and questions after discharge and documents in the medical record. * Other duties may be assigned Required Knowledge, Skills and Abilities * Must be a graduate of an accredited RN School of Nursing. * Must be licensed to practice professional nursing by the Board of Nurse Examiners for the State of Texas. * Six months (or equivalent) full-time nursing experience or BS in Nursing or related field preferred. * Must complete examinations and skills checklists within three months for new Scott & White hires. * Proficient in Microsoft Office, i.e., Excel, Outlook, PowerPoint.
Associate's Degree in Nursing
Texas Nursing License, BLS
2+ years of experience in an ICU or Med/Surg inpatient setting
