Texas NeuroRehab Center (TNC) provides complex medical and brain injury rehab, chemical dependency treatment for adults and neuropsychiatric treatment for youth. The capabilities of the facility as a whole allow for integrated and specialized care for children, adolescents and adults facing difficult challenges.
Texas NeuroRehab Center is currently accepting applications Case Manager. Case Manager coordinates patient care throughout the course of treatment for patients admitted to the program. Maintain communication system based on documentation and verbal exchange regarding treatment. Communicate with internal and external entities to facilitate acquisition of resources. Contribute to long-term planning. Assumes coordination and communication facilitation for treatment team. Assists discharge planning with nursing, therapists, patient, family, physician and unit Social Worker. The candidate selected will be responsible for the following: Exercises leadership in coordinating the treatment team in designing and implementing the patients on-going treatment plan and discharge plan. Ensures patient, family, and funding source involvement in the treatment process. Communicates with treatment team, patient, family and funding source regarding treatment priorities, discharge issues, and funding issues. Performs continued stay reviews, authorizations, appeals, and re-certifications, and completes additional recertification paperwork as required by the funding source. Completes miscellaneous paperwork as needed to assist with discharge planning, acquisition of aftercare funding, and other requests. Manages outside procedure costs effectively by utilizing the OMAR system of monitoring. Thoroughly documents all contacts in the patient's MTPRs.
