We are seeking a confident hospital pharmacy leader to step into this role as our Clinical Pharmacy Specialist. In this position, you will be an advanced clinical practitioner having additional expertise in a specific field of pharmacy practice such as infectious disease or Critical care. This individual will lead clinical specialized services offered by the department, you will be "boots on the ground" clinically speaking having direct patient interaction and yet mentoring a number of pharmacists while helping to develop various clinical initiatives at the hospital and participating in the P&T Committee meetings & their Antimicrobial Stewardship program. You will also be a part of the ASHP accredited PGY1 program and will provide educational activities to the pharmacy department, the healthcare team and the community as needed.
Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
Deliver medication therapy management within specialized patient care areas and ensure pharmaceutical care programs are appropriately integrated throughout the institution.
Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management.
Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.
The clinical specialist is also expected to be a apart of our ASHP accredited PGY1 program and should work in synchrony with the clinical team to provide educational programs for the healthcare team and the pharmacy personnel and the community if needed.
All applicants must have, at minimum, either a PGY1 Residency with relevant clinical experience or 5 or more years of relevant clinical hospital pharmacist experience. Applicant must hold one or more Board certification from the Board of Pharmaceutical specialty. TX license strongly preferred but not required to be considered.
