PeaceHealth is seeking a Disease Management Specialist - PHMG Quality & Coordination for a 1.0 full time equivalent Non-Exempt position. This position will be working Day shift with shift durations of 8 Hours .

Works directly with patients, clinicians, and staff to foster patient education and to facilitate the desired outcomes of the various disease management programs.
Analyzes data and makes recommendations based on criteria set forth by the appropriate disease management program committee and performance improvement committee(s).
Initiates lab orders based on expert rules and manages appointments to maximize patient outcomes and program effectiveness.
Oversees case tracking and interfaces with the client and medical providers.
Contacts patients in program to ensure that they participate and follow through with clinical guidelines and protocols
Facilitates changes in clinic processes that will promote the goals and objectives of the diabetes program.
Facilitates actions and/or referrals that will enhance patient knowledge and ultimately successful self-care behavior.
Performs other duties as assigned.
Associate's degree in a business or health-care related field OR equivalent education and/or experience which provides the incumbent the requisite skills to successfully perform the job.
Minimum of three years' experience with EMR (Electronic Medical Record) and/or EIPIC system as well as experience in an ambulatory care setting is required.
One year experience in coordinating projects/programs
Proficient with MS Office applications
Demonstrated ability to prioritize and manage multiple tasks at one time.
Demonstrated ability to promote positive relationships and a customer-focused work environment.
Demonstrated ability to work effectively within a self-directed teamwork environment.
Demonstrated ability to assist clinician with planning patient care.
PHMG PHYSICAL/COGNITIVE REQUIREMENTS:
Work requires fairly light physical exertion from 34 to 65% of the time.
Ability to lift objects weighing 30 lbs. or less.
Work requires frequent exposure to conditions involving minor cold, heat, poor ventilation or sharp instruments such as syringes or lancets. Reasonably anticipated exposure to blood and body fluids once per month or more.
Job duties frequently require intense concentration and attention to detail (34-65% of work time).
