This part-time (60%) Mental Health Family Advocate ("Patient Navigator 2") position (formerly Senior Community Health Program Representative) is for the Family Advocate Role in the Early Psychosis Programs (SacEDAPT Clinic) at the UC Davis Medical Center.The Family Advocate provides personal account of lived experience as a parent or direct caregiver of someone with serious mental illness to consumers, families, educators, community members and treatment providers. Participates in weekly clinic team meetings, assists with community outreach and education efforts, provides education and support to consumers and their families as part of a clinical team, and assists consumers and their families in accessing community resources. Services may be provided in a variety of settings, including the clinic, the community, local schools, or the family home. Through contact with consumers and families within SacEDAPT, the Family Advocate assists the treatment team in identifying individual, family, home or community conditions needing intervention and communicates the family's needs to the treatment team for intervention.REQUIRED--Willingness to share personal account of lived experience as parent or direct caregiver of someone with serious mental illness--Knowledge of community resources in Sacramento County--Previous experience supporting individuals or families in accessing supports or services--Completion of bachelors or associates degree --Previous part-time or full-time work experience --Excellent oral and written communication skills to effectively work with physicians, consumers and families, clinical staff, research staff, and other healthcare professionals --Demonstrated knowledge and experience with Windows based computers and associated software programs --Ability to work independently and manage multiple deadlinesPREFERRED:--Bilingual oral and written language skills --Previous experience working with adolescents and young adults with physical or mental health difficulties and their families (e.g. volunteer in hospital or clinic, experience in community program providing such supports or services)--Experience with public speaking--Experience with electronic medical recordsSPECIAL REQUIREMENT - THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
