The Transplant Coordinator III - Nurse Practitioner is the primary nursing resource and clinical expert in the care of pre-transplant candidate. The TC III NP has responsibility for the clinical case management of adult and pediatric patients from the time of referral until transplant. This responsibility involves coordination of the interdisciplinary medical care delivery system under the direction of the Medical Director. The Transplant Coordinator III - NP functions as the liaison between the transplant team and the patient, family, medical staff, referring physicians, and insurance companies. He/she provides transplant education to patients, dialysis unit staff, and community nephrologists.The Transplant Coordinator III - NP assumes responsibility for on-call activities including patient triage and coordination of deceased donor transplants. The TC III NP functions as a liaison between the transplant center and organ procurement agency. The TC III NP applies advanced patient assessment skills and knowledge in the area of immunology and tissue typing to perform on call functions.REQUIRED - At least 2 years of clinical Nursing Experience - Advanced case management, transplant, or nephrology experience - Master's of Science in Nursing - Possession of current California License as a Register Nurse - Possession of current California Nurse Practitioner License - Possession of a valid DEA number and furnishing license - Possession of valid NPI number - Passing of UCDMC Performance Demonstration, Policy Review and Medication Exam - Current BSL/CPR Certification PREFERRED - Certified Clinical Transplant Coordinator (CCTC) highly desirableSPECIAL REQUIREMENT - On-call duties - Must be able to work overtime and variable shifts when necessary - THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
