Provides direct and supervisory resident care according to physicians' orders and resident care plans, Supervises the Certified Nurse Aide's (C.N.A.) and Nurse Aide's on duty.
Minimum Education Requirements
High School Diploma or GED
Minimum Experience Requirements
1 year experience as LPN preferred
Current Colorado Practical nurse License (LPN)
Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from the American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without and AED.
Position Duties (Essential Job Functions)
Direct Care 60%
Provides direct care to residents according to each resident's overall plan of care, including medications and treatments.
Participates in the care planning process as assigned.
Provides instruction and education to residents and responsible parties regarding residents' condition and care practices.
Communicates changes in residents' conditions to the Charge Nurse, attending physician, responsible party and Director of Nursing in a timely manner.
Guides the activities of C.N.A.'s, Care Partners, and N.A.;s on duty, including monitoring of completion of their assignments and documentation.
Provides basic first aid for employees and visitors in the facility, including completion of incident reports.
Documents medications and treatments give, resident progress and condition changes, and other documentation assigned.
Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient care setting.
Thorough and confidential maintenance of patient charts and records.
Document ongoing status, interventions, patient response and outcomes in accurate, timely manner.
Use information technology to communicate, manage knowledge, mitigate error, and support decision-making.
Medium Work - Exert/Lift up to 50lbs of force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly.
