Job Posting: Summary of Essential Job Duties: The Clinical Program Coordinator (CPC) is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services—from initial referral to follow-up care in Heart Transplant. CPC attends transplant clinic (3 mornings per week), Selection Committee meetings, trains and rotates between Pre- and Post- Heart Transplant divisions, participates in primary and back-up on-call for 24 hours shifts several times per month for heart and lung, assists in lab review process, triages patient calls, and gets meds prior authorizations as needed. Serves as the heart transplant coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff. Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community. Collects/documents data to meet clinical, financial, and regulatory requirements. Participates in the development of guidelines, protocols, pathways, and other performance improvement activities. Participates in research activities. Serves as a patient advocate throughout the patient care process. Represents the Medical Center at regional and national meetings/conferences. The Clinical Program Coordinator provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, The Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan. He/she maintains competency skills including the ability to provide care to special patient populations and patient with diverse cultural backgrounds. He/she can identify the ability of the patients to follow directions/instructions and give consent. He/she can identify safety concerns and institute measures to provide a safe environment; and provide appropriate assessment, intervention and evaluation of patients with consideration of developmental milestones in the patient population.
Qualification Requirements/Preferences: Education Certifications/Licensure Experience Physical Abilities Educational Requirements: BSN degree required License/Certification/Registration Requirements: A current California RN License Specialty Certification Current BLS from the American Heart Association Experience: A minimum of 5 years of Nursing experience required At least 3 years of Critical Care experience in a Cardiac setting required Organ transplantation experience preferred Demonstration of excellent organizational skills as well as interpersonal skills Full command (verbal and written) of the English language Demonstrated knowledge of case management Experience in data management Physical Demands: (e.g., lifting, standing, walking) Sitting, walking, standing, bending, reaching, and lifting up to 25 lbs.
