The Nurse Practitioner will assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Under general supervision, incumbents perform operational level duties as described in the Series Concept. The major emphasis at this level is ongoing direct patient care delivery. Assignments at this level primarily encompass management of patients who present chronic and/or episodic health problems. Assignments at this level may include responsibility for occasional community teaching program's participation in program planning and development of patient education information.
The Nurse Practitioner will perform Annual Wellness Visits and E&M visits as needed on the Medicare Advantage population. These visits are recommended annually and help support member health maintenance each year. The NP will travel between primary care clinics. A health coach and HCC coder will help support this position by coordinating scheduling and updates on any gaps in HCC coding due to a refresh need based on clinical history and EPIC information obtained on patient during scheduled office visits. This position will be key to help educate our MA members and support MDs in achieving needed health maintenance for this population. The NP must possess a passion for patient education, strong assessment skills, and med management ability.
