Take this opportunity to start doing your life's best work.(sm) The Discharge Coordinator will work with Western State Hospital (WSH) to arrange follow-up and reentry care for consumers leaving intensive levels of care including inpatient and residential care. They will also assist in referrals to recovery oriented services and assist in making connections to allied systems supports and community support services. They will be responsible to ensure that effective discharge planning is implemented and linkages are made with the CMHA of the consumer's choice for follow up treatment services after discharge. This person must have a thorough working knowledge of resources available for consumers in Pierce County and will work closely and collaboratively with community and state hospitals staff, community providers staff, care managers, as well as consumers and their families relative to admission, treatment planning, and discharge planning. Primary Responsibilities: Use expert knowledge of the Systems of Care in Pierce County, along with knowledge of resources available Facilitate the return to the community for people at WSH through a collaborative process with WSH staff; attend all necessary meetings at WSH to facilitate discharge planning Conduct discharge planning to ensure adequate resources are available to provide stabilization for post hospitalization Participate in WSH treatment team meetings and discharge planning meetings Communicate with hospital regarding daily census alerts Be in contact with provider case managers and PCP's for consumers currently enrolled with a mental health agency when hospitalized Track all enrolled and non-enrolled acute Optum Health Pierce funded (Medicaid and Medicaid eligible) mental health inpatient admissions for referral and care coordination's needs Track and monitor all re-admissions Consult with Clinical Manager and/or Medical Director if there is a question of eligibility, medical necessity or a critical clinical circumstance (inpatient hospitalization, potential suicide risk, potential homicide risk, disagreement with provider about level of care, provider complaint or grievance). Refer all consumer complaints to QA/PI Manager Meet with Clinical Manager, Medical Director, and other staff as scheduled and/or needed to insure quality of care and appropriate utilization of services Provide training to contracted provider staff on a proactive basis by responding to provider requests and needs Perform Care Management functions as necessary and act as a liaison to contracted providers to facilitate admission of consumers to services
Required Qualifications: Bachelors degree 2+ years of experience in a managed mental health care environment Willing and able to travel up to 50% of the time within the Tacoma WA areaPreferred Qualifications:2+ years of experience with the Systems of Care in Pierce County, Washington, along with knowledge of resources available Advanced level of communication skills with all level Experience in a delivery system that follows the principles of recovery and resiliency Advanced level of skills in planning, coordination and relationship buildingCareers with Optum.
