You will be managing hospital admissions and coordinating their hospital/discharge transition. This will involve patient education, medication reconciliation, equipment reconciliation, self- symptom control education, inhaler use education, home equipment evaluation and case management. Primary Responsibilities: Manage hospital admissions and coordinate discharge transition Schedule post hospital discharge follow-up, by visiting patients in their homes and/or in the Pulmonologist's office Support patients with condition education and medication reconciliation Educate on self-symptom control and inhaler use education Equipment evaluation, as needed Hospice and Palliative care involvement Ensure and document patient goals of care, progress notes and other record requirements in the electronic medical record Provide a complete continuum of quality care through close communication with patients via in-person, in-office and via phone interaction
Required Qualifications: Current, unrestricted NV RN license or temporary NV RN license with the ability to obtain permanent NV RN license upon expiration of temporary NV RN license 3+ years clinical experience in hospital, critical acute care, or in complex case management Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications Access to reliable transportation that will enable you to travel to client/patient sites within a designated area Preferred Qualifications: Certified Case Manager (CCM) Pulmonary experience Bilingual skills Ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action Careers with Optum.
