FrontLine is commencing an initiative to provide a Central Intake and Rapid Re-Housing Program at both the Women's and Men's homeless shelters in Cleveland, to implement a homeless prevention and diversion service for young adults, ages 18-24, and to rapidly re-house shelter residents with a rental subsidy and case management follow-up.
Under the direction of the Program Manager the Rapid Re-Housing Case Manager will assist young adults in maintaining their short term subsidy, housing, and independence after they exit shelter. Rapid Re-Housing case managers will help young adults create, implement and achieve goals, obtain furniture and practical needs, obtain/maintain income, pursue educational endeavors, link with resources, and connect with natural and community supports.
Bachelor's degree with specialization in social, behavioral or human services and some direct care experience
